You run eLxr Server 12, a Debian-based enterprise Linux, on the EC2 instance type you select. The software itself carries no charge. One unit is one running EC2 instance, billed per hour based on that instance's size and family.
Am I charged when an instance running this software is stopped or powered off?
The software carries no charge in any state, since this listing is free. When an instance is stopped, no hourly software fee applies. You may still pay underlying AWS fees, such as storage for attached volumes. Running time drives the EC2 instance cost.
Why are there so many instance options, and how do I pick one?
Each option maps to a specific EC2 instance type and size. They are not feature tiers. General-purpose, compute-, memory-, storage-, and GPU-optimized families are all available. Match the instance to your workload capacity needs. The hourly rate reflects the instance chosen, not the software.

I had no problems creating and launching a VM and it boots very quickly. It's a good slimmed-down Debian 12 image with an upstream-first development methodology.
