Qualitrac is Telligen's proprietary, population health management application. This web-based, mobile friendly, HITRUST Certified system provides the flexibility and modularity required to handle todays complex healthcare data management and analytics solutions. Telligen offers multiple pricing and contract options.

Qualitrac includes a suite of four inter-connected modules that can function on their own or serve as a comprehensive population health management solutions when combined. We've designed Qualitrac so that it can seamlessly integrate with legacy Medicaid MMIS and Health Plan systems.

The four modules include: Case Management: In collaboration with case managers, provider, and members we have developed our Qualitrac Case Management module to support a comprehensive person-centered care coordination process by combining innovative technology and user-friendly design in a cloud hosted SaaS product that integrates easily into your populations health enterprise systems environment.

Utilization Management: Telligen's Utilization Management module efficiently manages the review process, promoting quality care and reducing unnecessary services. This solution eases the administrative burden for providers and staff, contributing to cost-effective healthcare management for our clients. Though integrated clinical criteria, we are able to automate authorization decisions, streamlining the process effectively.

Preadmission Screening and Resident Review (PASRR): Our innovative PASRR module empowers providers to effortlessly navigate the Level I screening submission process and features a workflow-driven approach for Level II assessments, seamlessly scheduling and conducting assessments while providing comprehensive outcome reporting and in-depth data analytics.

Quality Measures and Reporting: Our Quality Measures and Reporting module creates a pathway to collect, validate, enhance, and report quality data all in one place. Qualitrac includes a robust library of HEDIS and non-HEDIS measures for hospital, provider, and ACO quality reporting programs. Analytics and dashboards provide actionable insights to address gaps in care at all levels of aggregation, from the plan and population level, down to the provider and individual member results.
